32 Disease Management Program The Plan provides a Disease Management Program through Cigna called “Your Health First.” The Your Health First program offers extra support and resources for Plan participants with the following conditions: • Heart Disease • As thma • Low Back Pain • Coronary Artery Disease • COPD • Osteoarthritis • Angina • Type I or Type II Diabetes • Depression • Congestive Heart Failure • Metabolic Syndrome • Anxiety • Acute Myocardial Infarction • Peripheral Artery Disease • Bipolar Disorder If you, your Spouse, and/or your Eligible Dependent Child have one or more of the above conditions, a member of the Cigna team trained as a Nurse, coach, nutritionist or clinician, may reach out to you by telephone to get things started. Alternatively, you may call 855 - 246 - 1873 and enroll yourself in the program. If you choose to participate in the Y our Health First program, a team member who has expertise with your particular condition will talk to you, answer your questions, provide insight into your cond ition, and, where applicable, make recommendations for treatment, medications, and physician visits. Under the Disease Management Program, you will be connected with one dedicated contact person that will help you manage a chronic condition. The contact p erson may help you to create a personal care plan, understand medications or your physician’s orders, identify triggers that affect your condition, make educated decisions about your treatment options, and know what to expect if you need to spend time in a hospital. They may also help you to improve your lifestyle by coping with stress, becoming tobacco - free, maintaining good eating habits, and managing or losing weight. Your team member will work with you and your physician to monitor your progress throug hout your participation in the program. When you are doing well on your own, they can still assist you though a variety of on - line self - service resources to help you better understand your condition and overcome barriers to better health. The Your Health First program is offered without any cost to you. The program is totally voluntary, and completely private. Your personal information will be kept confidential, and it will not be shared with your Employer. The Y our Health First program meets all federal and state regulations, including those that are part of the HIPAA Privacy Act (see Confidentiality and Protection of your Health Information included later in this SPD). Note: Information and/or recommendations r eceived in connection with the Y our Health First program do not imply that you are eligible for the coverage under the Plan. You should contact Cigna, or the Fund Office directly, to confirm whether you, your Spouse, and/or your Eligible Dependent Child ha ve coverage under the terms of the Plan for any recommended services, or to get answers to any questions about the coverage provided by the Plan.
