The Plan may pay for purch ase of lactation equipment instead of rental, if deemed appropriate by the Plan; • Cervical cancer screening for women ages 21 to 65 with Pap smear every three years; • Human papillomavirus testing for women ages 30 and older with normal Pap smear results, onc e every three years as part of a well woman visit; • Chlamydia infection screening for all sexually active non - pregnant young women aged 24 and younger, and for older non - pregnant women who are at increased risk, as part of a well woman visit. • For all pregna nt women aged 24 and younger, and for older pregnant women at increased risk, chlamydia infection screening is covered as part of the prenatal visit; • For women of reproductive capacity, the Plan will cover at least one form of contraception in each of the FDA - approved contraceptive methods (including barrier and hormonal methods and implanted devices) as well as patient education and counseling, when prescribed by a health care provider. Services related to follow - up and management of side effects, counseli ng for continued adherence, and device removal are also covered without cost sharing. The FDA - approved contraception methods for women include:  Sterilization surgery (tubal ligation)  Surgical sterilization implant for women Implantable rod  Intrauterine dev ice (IUD) copper  IUD with progestin  Shot/injection  Oral contraceptives (combined pill)  Oral contraceptives (progestin pill)  Oral contraceptives (extended/continuous use)  Patch  Vaginal contraceptive ring  Diaphragm  Sponge  Cervical cap  Female condom  Spermicide  Emergency Contraception (Plan B/Plan B One Step/Next Choice)  Emergency Contraception (Ella) • Generic birth control prescriptions and brand name birth control prescriptions for which there is no generic equivalent; • The Plan may cover a generic dru g without cost sharing but charge cost sharing for an equivalent branded drug. The Plan will accommodate any individual for whom the generic would be medically inappropriate, as determined by the individual’s health care provider and the Prescription Drug Coverage prior authorization process;
