47 • Alcohol and drug use assessmen ts for adolescent • Critical congenital heart defect screening in newborns; • Hematocrit or Hemoglobin screening for children; • Lead screening for children at risk of exposure; • Tuberculin testing for children at higher risk of tuberculosis; • Dyslipidemia screeni ng for children at higher risk of lipid disorders; • Sexually transmitted infection (STI) screening and counseling for sexually active adolescents; • Cervical dysplasia screening at age 21; and • Oral health risk assessment; • Newborn screening tests recommended b y the Advis ory Committee on Heritable Disorders in Newborns and C hildren (such as hypothyroidism screening for newborns and sickle cell screening for newborns); • Prophylactic ocular topical medication for all newborns for the prevention of gonorrhea; • Oral f luoride supplementation at currently recommended doses (based on local water supplies) to preschool children older than six months of age whose primary water source is deficient in fluoride. A prescription must be submitted in accordance with Plan rules; • I ron supplementation for asymptomatic children aged six to 12 months who are at increased risk for iron deficiency anemia; • Obesity screening for children aged six years and older, and counseling or referral to comprehensive, intensive behavioral interventio ns to promote improvement in weight status; • HIV screening for adolescents ages 15 and older and for younger adolescents at increased risk of infection; • Counseling for children, adolescents, and young adults ages 10 to 24 years who have fair skin about min imizing their exposure to ultraviolet radiation to reduce risk for skin cancer; • Screening for H epatitis B virus infection in adolescents at high risk for infection; and • Application of fluoride varnish to the primary teeth of all infants and children up to age 5 starting at the age of primary tooth eruption, in primary care practices. Immunizations • Routine adult immunizations are covered for you, your spouse, and dependent children who meet the age and gender requirements, and who meet the CDC medical criter ia for recommendation. Immunizations administered for travel are excluded. • Immunization vaccines for adults — doses, recommended ages and recommended populations must be satisfied:  Diphtheria/tetanus/pertussis;  Measles/mumps/rubella (MMR);  Influenza;  Huma n papilloma virus (HPV);  Pneumococcal (polysaccharide);
